A middle-aged male patient with a hunch back underwent surgery for removal of his gall bladder at Sharda Hospital. The patient had a medical condition called Ankylosing Spondylitis in his spine which had lead it to be fused & bent along with his neck. The patient was unable to lie straight on his back for the surgery.
Managing such cases for surgery is a very challenging task as it is difficult to make the patient lie down for adequate surgical procedure and there is a risk of fracturing the fused spinal bones which may even result in an injury to the spinal cord. Secondly, for such surgeries, general anaesthesia administration requires anesthesiologists to control patient’s airway & breathing while he is sedated. This again can be very challenging for such patients, as the neck is firm & the anaesthesia can only be given to the patient in a half-sitting position. The patient’s lungs had also partly collapsed due to bent & rigid chest wall.
Keeping the above conditions in mind, the team of doctors prepared the patient for routine surgery and 3-4 pillows were placed under the head of the patient in such a way that the patient was comfortable and could easily be put under general anaesthesia. Securing the airway of the patient with an ET Tube was a difficult task, but was successfully managed with modern airway devices & expertise of the anaesthesia team, lead by Dr Ritika Srivastava, Assistant Professor under HOD, Dr Suveer Sharma. The surgery was performed by Dr P.L. Karhialu, Senior & Onco Surgeon at Sharda Hospital.
The surgical procedure was a success & the patient was provided with optimum post-operative care to make sure that he was in a comfortable & pain-free position with no recollection of surgical or perioperative events. The patient was discharged and sent home the next day of surgery.
